Montana Index for

Healthy Communities

RESOURCE HUB

Welcome to the Montana Index for Healthy Communities (MT IHC) Resource Hub!

The MT IHC is a health and social needs index that quantifies the social, economic and healthcare-related factors that influence the health of Montanans. This hub is your source for accessing data, important resources, and answers to frequently asked questions.

Social Factors

Historically marginalized populations, education, household structure, food access and more.

Economic Factors

Employment, income and poverty, housing and transportation, marginalized people and more.

Healthcare-Related Factors

Disease prevalence and prevention, health insurance, health behaviors, and more.

Why use a social needs index?

Many health outcomes are associated with non-clinical factors such as access to stable housing or healthy food. A social needs index like the IHC can identify opportunities to improve health through investing both within and beyond the healthcare delivery system. Data can show you domains with high levels of need (such as food insecurity, internet accessibility or provider availability) as well as the geographic areas with highest levels of need. This data will help your organization prioritize investments, partnerships, or programs with the potential to have the greatest impact on improving the health of your community.

  • The partnership with Valley County Transit has been amazing. We uncovered many things that we didn't know about each other and how we can partner to best serve the patients and residents of the community.
